All "no" answers should be accompanied by an explanation (space provided at the bottom of this form) as to why the equipment start-up requirement is not available, so the Service Department can determine if a proper start-up can be provided. We will be happy to reschedule a start-up if the equipment is deemed to be unprepared for start-up.
1. Is the equipment undamaged?
2. Have all mechanical connections been made to the equipment?
3. Is the equipment base mounted and secured?
4. Is the equipment or pump base grouted?
5. Has the hydronic system been filled?
6. Is make-up or domestic water connected and available?
7. Has the hydronic system been vented and checked for leaks?
8. Is steam available at the appropriate pressure and capacity?
9. Have all electrical power connections been made and is power available?
10. On pneumatically controlled devices is air connected and available?
11. Have all BAS connections been made and is the BAS system operating?
12. All external trim components installed and connected back to the equipment?
13. Can a load be established on the equipment for capacity verification?
14. Is the equipment ready to operate?
Please explain any "no" answers. Be sure to indicate the question number in your comments.
Please notify us 24 hours before our scheduled visit if any of the above requirements have been determined to be unavailable or incomplete. We will reschedule the start-up at no additional charge. If Bornquist Service Personnel arrive at the equipment location and the above items have not been completed, we will leave instructions as to what is missing, and also fax this information to you. We will then reschedule the actual start-up for another day and time at your convenience. A one-hour charge of $95.00 will be assessed for the unsuccessful start-up. I hereby certify that the above items have been completed and the equipment will be in a "ready-to-run" condition prior to the scheduled time that Bornquist personnel will arrive to perform start-up.
